Program Booking Agreement

Patrick Mathieu UNLIMITED's Program Booking Agreement details the terms and conditions of the agreement between your organization and Patrick Mathieu UNLIMITED. This includes:

• Date of Event(s)
• Time of Event(s)
• Location of Event(s)
• Travel Arrangements
• Accommodation Arrangements
• Expenses
• Cancellation Policy

Your Organizational Profile

Patrick's goal is to ensure your event participants receive the maximum impact. Therefore, Patrick always works with you, the event planner, to customizes the message to fit with your objectives. Whether Patrick is delivering a keynote speech, workshop or both, we always ask you to fill out an Organizational Profile in order to give us a better understanding of your organizational culture, business, challenges, and successes. This information is used by Patrick to tailor his message to your particular organization.

Pre-Event Questionnaire

Patrick Mathieu UNLIMITED uses a Pre-Event Questionnaire to completely understand the objectives you have for your event. This questionnaire collects information on the event theme/purpose, audience members, group dynamics, timing of the event, and other relevant information. The use of this Questionnaire makes your job as Event Planner easier by ensuring that Patrick has a crystal clear understanding of the event. Where appropriate, Patrick likes to arrange to have a quick interview (phone or e-mail) with two or three of the event participants in advance.

Logistics

To book Patrick, here are the appropriate steps:

1. Contact Allan Todd to check Patrick's availability and discuss fee structure.
2. Allan will send you a Program Booking Agreement, Organizational Profile and Pre-Event Questionnaire.
3. Return the completed Program Booking Agreement and 50% of the fee, which is used as a deposit to hold the date(s).
4. Return the completed Organizational Profile and Pre-Event Questionnaire
5. If appropriate, Patrick will interview event participants
6. One week before the event(s), all details will be re-confirmed.
7. The remainder of the fee is due on the date(s) of the event(s)
8. ENJOY THE PROGRAM!
9. Patrick will leave you with a feedback form.
10. If appropriate, follow-up sessions will be scheduled.
